This toolkit is designed for health professionals working with survivors of stroke. It provides extensive assessment tools and resources to support the development of tailored exercise and physical activity programs.

 

It is designed for the health professional and the survivor of stroke to work together, aiming to develop programs that are adhered to, leading to improved long term physical activity.

THE TOOLKIT

To begin using the toolkit, the first step is to register.

 

A toolkit flowchart will then assist you in selecting the appropriate assessments and planning documents tailored to your needs. Additionally, the toolkit is equipped with downloadable resources, ensuring easy access to resources throughout the process.

Dr Tamina Levy

Dr Tamina Levy is an experienced Adelaide-based physiotherapist, educator, and researcher. ​In her PhD Tamina explored exercise adherence in survivors of stroke and since then she has been passionate about promoting adherence and behaviour change in this population, and empowering health professionals working with survivors of stroke.

 

Tamina is proud to have led an amazing team of researchers, health professionals, and survivors of stroke in developing the ACT Now Toolkit, which the team hopes will contribute to improve long-term health outcomes in this population.
